1. Who is covered under these plans?
The Plan covers you, as the primary member,
plan member’s significant other and unmarried dependent children household who are under the age of 23 and any categorically dependent individuals living in the plan member’s home.

2. How soon can I begin using the benefits in the membership?
In most cases, only 48 hours or less. Once you signup online, simply forward us the Confirmation # provided at the end of the application form. We can then quickly process your account for immediate use.

3. Do I have to become an authorized agent in order to join this plan?
No, becoming an authorized agent is a voluntary Income Opportunity.

4. What payment options are available?
You have the option of paying by: Bank Draft (Checking or Savings) or Credit Card (Visa, Mastercard, Discover and American Express). You may use a debit card if it has a major credit card (i.e. Visa, Mastercard, etc.) symbol on it.

5. Are there any contracts or terms in order to join?
No, there are no contract terms. You can cancel, downgrade or upgrade your plan at ANYTIME.

6. What is the difference a Member and Authorized Agent?
A Member is an individual or business owner who enrolls in a plan to use the benefits in regard to their legal matter or other benefits.

An Authorized Agent is an individual or business entity who wishes to generate revenue by selling the service plans.

9. Why should I become a member?

When it comes to qualifying our attorneys, we have the strictest standards in the industry. Less than 10% of all law firms contacted, actually qualify to become part of our network. When your life or your families financial future is on the line, who would you rather have on your side, someone in the top ten percent of the nation, or someone in the bottom 90%?

All of our providers have been credentialed, with verification of licenses, degrees, insurance, and a clear standing with local regulatory agencies. In addition, we have profiled them by range of practice, so you will know you are being referred to the right professional who is qualified to work with you.

1. Can this plan assist me with my legal problem?

2. What areas of law do the attorneys practice in?
The network of attorneys practice in each area of law from child custody, divorce to criminal law (including all others).

3. When I call for an attorney, what will happen?
When you need an attorney, you call the member only toll free number. You then provide the customer service representative with your membership number. Your matter will be assigned to an attorney according to your location and area of law that you need assistance in. You can then discuss your legal situation or questions with one of over 20,000 qualified attorneys.

4. Can I choose my attorney?
Your membership provides you with a network of attorneys to assist you with your legal needs. We take the hassle out of searching for the appropriate attorney for you.

5. Can I see a list of attorneys in my area?
Each state has a referral network of attorneys in place to assist you. If for any reason that attorney firm is not able to assist you, such as location, or area of law, they will refer you to an appropriate attorney. The attorney network in most states is a Proprietor Network, and the list is not published.

6. How do you select your attorneys?
Subject to a careful screening process, attorney candidates must qualify within a set of guidelines before they will be considered. We evaluate their standing with State Bar or the appropriate attorney regulatory authority.

7. Can my attorney handle business matters under the Legal Only plan?
Yes. The plan covers both personal and business legal matters.

8. Can I e-mail my legal questions or matters to this site?
No. In order to provide you with the best service, our attorneys require a more fluid form of two-way communication. We are developing some additional online resources for the future, but right now we are able to offer consultations either over the phone or in person.

Once you become a member, you can consult with an attorney in your area in regard to your matter in addition to utilizing all the plan benefits.

9. Will I have access to an 800 number for matters with my account or legal help?
Yes. When you need to update your account or have question on your account, you can access the member toll free number for assistance. Members also have access to a toll free number to request assistance from an attorney.

10. What do I have to pay in addition to my monthly fee?
Participating in the plan does not mean you will never pay legal fees again. The program covers the most frequently needed and used legal services. Should you need to retain the attorney for representation, you will be asked to sign an agreement and pay a retainer fee. This fee varies based upon the case and its merits. The program covers legal fees. Fines, court costs, document and filing fees are additional.

11. How soon will an attorney call me back?
You should expect a return call from an attorney within 24 hours. Please be prepared to give the receptionist a couple of different phone numbers where they can reach you. If you do not hear from the attorney in a day, give them another call.

12. Are the legal forms valid?
These forms are for information purposes only. Laws and procedures change frequently and are subject to differing interpretations. We therefore cannot guarantee that they are up to date and appropriate for your particular situation.

That is one of the reasons why we offer our members a free one-hour consultation with an attorney, so that you can discuss your situation and get the appropriate advice.

13. What if I am not comfortable with the referral and want to talk to a different attorney?
If we provide you with an attorney that you are not comfortable with for any reason, we will provide another attorney referral.

Yes. You can use the plan for pre-existing as well as future needs.

15. What if the attorney you refer me to will not take my case?
If we refer you to an attorney who is not able to provide assistance, we will find another attorney for you. We can also assist you in discussing your situation with the attorney to make sure that you receive the care and attention you deserve.

16. Do I have to retain the attorney in order to get my questions answered?
No. Actually, our statistics indicate that up to 85% of our callers are able to resolve their issues during their free initial consultation.

17. How does the plan work?
It’s easy to use; simply call our toll free number to access any of our legal or financial services. Present the customer service representative with your name, membership number, and the type of issue you have. For an attorney referral, you will be provided with the name, address and telephone number of the provider firm located nearest you. If it is regarding a financial issue, you would present the Customer Service Representative with your name and membership number or case number and the financial question you have. You would then be connected with a Financial Specialist who will provide a telephone consultation or a local referral when appropriate.

Each member is entitled to one (1) free office consultation or telephone consultation per separate legal or financial matter. In the event you retain the attorney after the initial consultation, the attorney has agreed to provide their services at a discount of 25% from their normal hourly rate. In the event a local referral is indicated for a financial issue, the provider will generally provide a 25% discount as well.

18. How many times can I talk to an attorney for free?
There is no limit to the number of initial consultations you request. You are able to speak with an attorney at no cost, for up to one hour, for each new legal matter.

1. Can I discuss my debt or credit card questions with a financial specialist?
Yes. Our financial specialist will assist you with questions regarding debt, credit card matters and budgeting. The scope of our financial support is very broad, covering most issues.

2. Can you assist me with college planning, retirement planning, or general investment questions?
Yes. Once you have outlined your financial issue to us, we will put you in contact with our financial specialist. In the discussion, your specific issue will be identified and reviewed. If it is determined that further action is necessary, a referral will be made to one of our financial partners who is able to prepare specific analysis or discuss your situation in detail.

3. Can you assist me with income tax issues?
We can assist you with questions or issues relating to income tax matters. We have a CPA on staff that can address your questions and also prepare your personal income tax return at a discount. This tax preparation service is telephonic with all materials sent via the mail or fax.

4. What financial support services are included in the plan?
Financial assistance is available on several different levels in our benefit package. Services range from individual consultations involving budgeting, credit matters, estate and retirement planning, and college funding, to various investment subjects and tax issues.

Health - Questions & Answers

1. What if there are no doctors in my area?
If there is not a participating provider in your area, talk to them about Liberty and ask if you can have information sent to them. Then you can send us your doctor/dentist name, address, and phone number, or go on-line and enter a provider nomination. We will contact them about becoming a provider.

2. Do the Plus benefits coordinate with other insurance a member may carry?
Yes. Pay the discounted rate at the time of services and file a claim with your current insurance carrier. It will be up to you, the member, to file such claims.

3. Will the discounted rate always be the cheapest rate for my prescription?
Not always. The discounts are figured on "National Average Retail Price" for that drug. Your pharmacy may already have rates discounted at or below the average cost. However, this will not be the case on all prescriptions.

4. If I have a problem with one of the service providers, who do I call?
You would contact the Customer Service Department of the vendor of whom you are having problems. For example, if you are having difficulties with the legal services, contact the Liberty Legal Customer Service Department. Your membership handbook will list each vendor's Customer Service Number.

5. What do I say when I present my card or call for benefits?
On the back of your card your will see a listing of our national network providers. When you present your card or call for benefits, please state two things:

1. Mention you are a premier discount cardholder.
2. Mention the name of the National network related to the benefit you are requesting.

For example: When filling a prescription at your nearby pharmacy, be sure to mention you are an cardholder and a part of the MIM Health System network.

6. When should I use the mail order facility?

You'll enjoy the best possible savings on drugs you use day in and day out (maintenance drugs) that are usually prescribed for 90 day of treatment or more. Mail order offers the best savings possible on generic drugs.

7. How do I transfer prescriptions?
If you are already taking a medication, call your doctor to request a new prescription for the maximum days supply allowed, call and get a patient information sheet and mail it in with your new prescriptions.

8. Are the providers well known?
Absolutely, for prescriptions drugs, virtually all the major chains are members, and many of your neighborhood pharmacies. For example: Wal-Mart, Walgreen's, Eckerd, Albertson's, PharMore, Kroger, Target and many more. Also, the vision network has well-known participants: Lens Crafters, Pearle Vision, Eyemasters, Vista Optical, Sears and many more regional chains and independent practitioners.

9. How do I know where the nearest provider is for all the services?
When you receive your card and membership handbook, we will supply a toll-free number and Internet address to locate the nearest provider to you. You can also use the Liberty web site to do a Benefit Provider search.

10. Where generally are your dentist more prevalent?
Urban areas. People in rural areas will usually have to drive to a larger community to secure dental care.

11. How does the dental benefit work?
Like all of our benefits, the dental benefit carries a cash, on-the-spot discount with each purchase. You simply present the card to anyone of our participating providers and you'll immediately receive your discount. Just choose one of the participating dentists you want to use from the list on the web site or call the toll-free number for a list.

12. How can my doctor become a part of the Network?
You can nominate your doctor by completing the "Nominate a Provider" form once you become a member.
